What Makes an Application Complete & PTBC Disclosures
Graduate from an Accredited PTA Program
Complete Application Consists of the Following Documents
- Fee Schedule with Appropriate Application Fee(s)
- Application answer all questions, staple or paste picture, sign and date in blue ink. DO NOT send a copy of your social security card.
- Verification of Clinical Experience Form (V1)
- PTA Equivalency Application Coursework Checklist completed and submitted with one copy of official transcripts in a sealed envelope and the second copy of transcripts the courses highlighted.
- Completed Agency Copy of Live Scan Form or a completed PTBC Fingerprint Card.
- If you require reasonable accommodations for the examination, a completed Disability Accommodations Request Form (D1) & Instruction for Evaluators must be completed and submitted with the application.
