Endorsement/Verification Letter

Steps to Requesting License Verification to another Jurisdiction

The Endorsement/Verification Letter the PTBC will send to another jurisdiction is the same information that is available through the PTBC's Online License Verification.

1. Prior to submitting your request to the PTBC, contact the State in which you are applying for licensure to determine if that Physical Therapy Board or Agency will verify your California PT or PTA license under Verify a License on the PTBC's website. If the State Board or Agency requires an Endorsement/Verification Letter then proceed to Step 2 & 3.
   
   
2. Submit in writing a request to the PTBC to send an Endorsement/Verification Letter of your PT and/or PTA license to another jurisdiction.

   Your request must include the following information.

   • Full Name
   • PT and/or PTA License Number
   • Current Address
   • Phone Number
   • Jurisdiction(s) in which you are applying
   • Signature
3. The fee is $30 for each Endorsement/Verification Letter. If you are requesting more than one Endorsement/Verification Letter, please submit $30 for each request. Submit a check or money order made payable to the PTBC along with your written request.

In order for the PTBC to overnight your request (i.e. UPS, FedEx, etc) to the other jurisdiction, a prepaid overnight envelope must be submitted along with your request.

Please allow the PTBC up to three (3) weeks to process your request. The PTBC will not expedite your request as the letters are processed in the order they are received.
